hile infrequently talked about, cosmetic surgery on the female genitalia has risen in popularity in many parts of the country. More and more women are bothered by an enlarged or protruding appearance of the labia minora. Most commonly, this is the result of congenital enlargement that can worsen with hormonal changes seen during puberty, childbirth, or aging. Several social factors have contributed to an increased awareness in what many consider ”normal” appearing external genitalia. Just like other aesthetic differences, this can result in diminished self-esteem and make one self-conscious about their appearance. In addition, many patients with enlarged labia complain of discomfort, irritation, hygiene issues, and problems wearing certain clothes. In fact, most women who seek this consultation are equally bothered by aesthetic concerns and physical discomfort. As a result, the surgical reduction in the size and length of the labia has become a procedure more often discussed in both the press and among plastic surgeons.
Surgical correction for the enlarged and protruding labia, also called labiaplasty, has been performed for quite some time, but refinements in the techniques have been made in response to the new interest in the procedure. Like most aesthetic procedures, patient selection involves the assessment of one’s complaints, having achievable and realistic goals, and the overall health of the patient. The procedure is typically performed under a twilight or sedation anesthesia on an outpatient basis. Depending on the individual, different types of incisions are chosen to remove excess skin and to reshape the labia. It is generally well tolerated and patients can return to normal activities in a few weeks. Risks include scars that may be thick or tender, asymmetry, and stretching. However, reoperation to correct any problems is rarely needed. The use of appropriate technique in well-selected patients has resulted in a high postoperative satisfaction rate for patients regardless of whether they seek improvement in appearance or to alleviate discomfort.
While far less common than other cosmetic procedures like liposuction, breast augmentation and tummy tucks, labiaplasty appears to be an emerging trend. Because plastic surgeons have experience in the processes required for reconstruction following gynecologic cancer surgery and trauma, operating on the female genitalia is not unusual. Aesthetic and functional concerns can be successfully addressed with the correctly planned procedure resulting in high rates of patient satisfaction.
